Electronic Records

Once you have completed your records, you will be able to access your account to schedule future appointments and/or send us messages.  To do so, click here.

Instructions for Completing Your Electronic Health Record

1. You will receive an email from the Chiropractic Café with the subject "Your New Electronic Health Record"
2. Open the link in the email, enter your name and the PIN (which you will be given when you schedule your appointment).
3. Complete the "Security Check Point" tab
4. Check your work and then click Continue
5. You will then receive an email activation link to your email account within a few minutes of completing this portion.  The subject will be "Patient Activation"
6. Click the link in the email and follow the instructions to log in
7. Click on "My Profile", and please complete: Health History & Update Emergency Contact
   
8. Under Required forms, please complete "New Case Intake"

Once you complete all of these items, you will be finished and ready for your first visit. If you have any questions please call and we will help. This will take about 10 minutes to complete.
